Instructions Relating to the Forms & Documents Section of the Conversion Dashboard

RPG Executed Agreement - Save a copy of the fully executed RPG contract for your records.

Termination Letter(s) to Prior TPA/Recordkeeper - Termination letter(s) should be sent to the prior provider(s) to inform them of your decision to terminate their services. Please draft the letter on your Firm’s letterhead, provide your signature and email to your prior provider(s). Please request a confirmation acknowledging their receipt of the email/letter and forward to us any communications you receive from them.

Driver’s License – Please provide a copy of your Driver’s License (to be used as sponsor identity verification) via the Secure File Upload link

Custodial Agreement - Please review, sign and return the agreement via the Secure File Upload link on the Dashboard page or via HelloSign.

RPG Electronic Deferral Election Authorization - E-deferrals are an easy way for participants to choose contribution amounts. Click here for a detailed description of the electronic deferral election process. If you would like to enable this feature, kindly sign and return the agreement via the Secure File Upload link on the Dashboard page or via HelloSign.

RPG ACH Authorization Form - ACH is our most common form of payroll deposit transmittals. If you choose to activate this option, RPG will automatically initiate an ACH request to pull funds from your business account upon receipt of the payroll contribution file. Please review, provide the bank account information, sign and return the agreement via the Secure File Upload link on the Dashboard page or via HelloSign. 

Custodial Authorization for Direct Payment Form - If you are enabling the ACH option, please review, provide the bank account information, sign and return the agreement via the Secure File Upload link on the Dashboard page or via HelloSign. 

Other Action Items

Contact Management - Follow the Plan Contact Roles link on the Dashboard page to assign each plan contact from your Company/Organization who will handle plan administration responsibilities or have access to plan information.

Documents – The Documents Required section on the Dashboard page lists the items that are critical to setting up the plan, and we request that you (or your prior provider) submit them as soon as possible.

ERISA Bond - If you already have an ERISA Bond, please provide a copy. If you need to obtain one, please refer to our website for more information.
